Step 1. Fill out the following two forms to redirect your tax dollars to GOAL.
1. The Woodward Academy Georgia GOAL Contribution Form.
2. A blank copy of page 2 of the FORM IT-QEE-TP1 pre-approval form, signed by you at Part C.
3. A check made payable to “Georgia GOAL Scholarship Program, Inc.” or credit card payment in the amount of your contribution. Be sure to designate Woodward Academy in the memo section of the check.
Step 2. Send the two forms and your check directly to Georgia GOAL at:

If paying by credit card, you may mail or fax the two forms to Georgia GOAL's secure fax number: 877.478.4625 (877.478.GOAL).
Step 3. Claim your credit.
When you file your income tax return in 2012, you will receive a credit against your Georgia income taxes for the amount of your contribution AND a charitable deduction on your federal income taxes, provided you itemize. You may still file your returns electronically if you elect to take this credit.
- GOAL will send you a Form IT-QEE-TP1 contribution acknowledgement form to be filed with your taxes.
GOAL will send taxpayer filing guidance early next year and/or will post taxpayer filing guidance on its website.
